Registration Process

At least 3-5 days prior to your scheduled procedure date, please access our website to complete the Patient Medical History Form. A Registered Nurse will review your information and may contact you if we have any questions about the information you’ve provided. If you prefer to give your information over the phone you may contact one of our Pre-Op RNs at 256-772-6014 between the hours of 8 a.m. and 4 p.m. Monday-Friday.

A member of our Business Office staff will contact you by phone to inform you of any financial responsibility and to answer any insurance or financial concerns you may have.  You may also call (256) 319-9000 during business hours with any questions. Our primary goal is to provide you, our patient, with quality healthcare in a friendly, efficient, and cost effective manner.

*Important information for patients taking GLP-1 antagonist medications  — PLEASE READ the following:*

From our Medical Director:

If you are a patient taking one of the GLP-1 antagonist medications listed below you are at an increased risk of peri-procedure regurgitation and aspiration given the drug’s associated delay in gastric emptying. Until we have more guidance and experience with these at-risk patients, we will mandate that the weekly injectable GLP-1 agonists be held 7 days prior to the day of procedure. Any patient that has not followed this guidance could be subject to rescheduling. Please notify your MD if you are taking one of these medications so your procedure can be scheduled at least 7 days after your last dosage. See below for a list of these medications:

• Dulaglutide (Trulicity)
• Exenatide (Byetta)
• Exenatide extended release (Bydureon BCise)
• Liraglutide (Victoza, Saxenda)
• Lixisenatide (Adlyxin)
• Semaglutide (Ozempic, Rybelsus)
• Albiglutide (Tanzeum)
• Tirzepatide (Mounjaro)
• Wegovy (semaglutide)
