For your convenience, you may complete your pre-registration form online.

You should have received a link vie text or email to our online registration portal.  If you need us to send you a new link, please email preop@madisonsurgerycenter.com and we will provide a new one.

A Registered Nurse will review your information and may contact you if we have any questions about the information you’ve provided. If you prefer to give your information over the phone you may contact one of our Pre-Op RNs at 256-772-6014 between the hours of 8AM and 4PM Monday-Friday.

*Important information for patients taking GLP-1 antagonist medications  — PLEASE READ the following:*

From our Medical Director:

If you are a patient taking one of the GLP-1 antagonist medications listed below you are at an increased risk of peri-procedure regurgitation and aspiration given the drug’s associated delay in gastric emptying. Until we have more guidance and experience with these at-risk patients, we will mandate that the weekly injectable GLP-1 agonists be held 7 days prior to the day of procedure. Any patient that has not followed this guidance could be subject to rescheduling. Please notify your MD if you are taking one of these medications so your procedure can be scheduled at least 7 days after your last dosage. See below for a list of these medications:

• Dulaglutide (Trulicity)
• Exenatide (Byetta)
• Exenatide extended release (Bydureon BCise)
• Liraglutide (Victoza, Saxenda)
• Lixisenatide (Adlyxin)
• Semaglutide (Ozempic, Rybelsus)
• Albiglutide (Tanzeum)
• Tirzepatide (Mounjaro)
• Wegovy (semaglutide)
