PATIENT'S BILL OF RIGHTS

The Madison Surgery Center adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Under HIPAA guidelines patients are provided confidentiality and privacy regarding their individually identifiable health information and informed when or with whom that information may be shared.

  • A patient has the right to be treated with courtesy and respect, and with appreciation of his or her individual dignity.
  • A patient has the right to privacy of information given and services provided.
  • A patient has the right to know who is providing medical services and who is responsible for their care. Patients have the right to be informed of any persons other than routine personnel that will be observing or participating in their care.
  • A patient has the right to know what patient support services are available
  • A patient has the right to be given by their health care provider information concerning diagnosis, planned course of treatment, alternatives, risks, and prognosis.
  • A patient has the right to review and obtain a copy of their health information, unless their provider believes access could cause harm to the patient or another. In such situations the patient has the right to have the denial reviewed by a licensed health care professional for a second opinion.
  • A patient has the right to amend their health information if that information is inaccurate or incomplete.
  • A patient has the right to be given the opportunity to participate in decisions regarding their health care unless such participation is contraindicated for medical reasons.
  • A patient has the right to refuse any treatment, and be informed of the consequences of those actions, except as otherwise provided by law.
  • A patient has the right to be informed of the facility's policy on advance directives and living will.
  • A patient has the right to receive a copy of a reasonably clear and understandable, itemized bill and, upon request, to have the charges explained.
  • A patient has the right to treatment for any emergency medical condition that will deteriorate from failure to provide treatment.
  • A patient has the right to know if medical treatment is for purposes of research and to give his or her consent or refusal to participate in such research.
  • A patient has the right to decide whether or not their health information may be used for marketing or advertising purposes.
  • A patient has the right to express grievances regarding any violation of their rights, through the grievance procedure of the health care provider or health care facility which served him or her, and with Department of Health and Human Service.
  • A patient has the right to know whether or not protected health information has been furnished to the police or public health authorities (in applicable circumstances) as required by law
