PRE-OPERATIVE INSTRUCTION SHEET
The Madison Surgery Center adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Under HIPAA guidelines patients are provided confidentiality and privacy regarding their individually identifiable health information and informed when or with whom that information may be shared.
- DO NOT eat or drink anything 8 hours before your surgery. (This includes water, mints and gum)
- DO NOT smoke or use any tobacco products for 8 hours before surgery.
- If you are a diabetic follow your physician's instructions about your medications.
- If you take heart, blood pressure, seizure, reflux, or asthma medications, please do so with a small sip of water, unless otherwise directed by your physician. Bring medications and inhalers with you to the center.
- You MUST have a responsible driver. The driver MUST stay at the surgery center for the entire procedure. Your procedure WILL NOT begin without a driver present.
- Bring your driver's license and insurance card with you to the surgery center. If co-pay is required, please bring a payment method.
- Please do not bring any valuables with you for your procedure.
- If you have not had a hysterectomy or tubal ligation, a urine specimen will be obtained.
- You must remove all jewelry before your procedure. (This can be a hazard during your procedure)
- Remove all nail polish.
- Remove contacts.
- You may brush your teeth the morning of your procedure.
- Wear loose fitting clothing. If you are having an upper endoscopy or surgery, you may wear cotton underwear and socks. Nylon will not be allowed.
- Please bring a list of all medications, including dosages you currently take with you the morning of your procedure.
- For pediatric patients, please bring a bottle or sippy cup for them to have after the procedure. They are also allowed to bring a comfort toy.
- For cataract patients, please wear short sleeved shirts that button down the front.
- Due to space limitations please do not bring more than two visitors with you. (Pediatric patients please bring parents/guardians only.
- If applicable bring guardianship, court documents related to custody of pediatric patients. Procedures will be cancelled without official proof of custody.
- If you have not been contacted by our pre-op caller, please call
256-772-6014 to give a current medical history.
- If you have an allergy to latex please alert the pre-op caller.
